UPMC’s Post-Acute and Senior Care provider team partners closely with Amoroso Woodridge Healthcare & Rehabilitation to deliver consistent, on-site medical care across levels of residency, including short-term rehabilitation, long-term care and respite care.

Our team delivers comprehensive medical management, including ongoing condition monitoring, medication optimization, and early identification of clinical changes. We focus on smooth handoffs from hospital to facility and across levels of care, helping reduce avoidable hospitalizations, improve outcomes, and ensure continuity of care for residents and families through every stage of their stay.

Krishna Nemani, MD

UPMC Post-Acute & Senior Care

Dr. Nemani specializes in geriatric medicine and is board-certified by the American Board of Internal Medicine.

Dr. Nemani received his medical degree from Ross University School of Medicine in Portsmouth, Dominica and completed residency programs at One Brooklyn Health Brookdale University Hospital & Medical Center and Seton Hall University School of Health and Medical Sciences.
